Prenatal Chiropractic Care: What to Expect During Each Trimester

Pregnancy changes the way your body moves, carries weight, and responds to everyday activity. As the uterus grows and your center of gravity shifts, it is common to notice lower-back discomfort, pelvic tension, hip pain, or changes in posture. Prenatal chiropractic care is designed to respond to those changes with positioning and techniques adapted to the stage of pregnancy.

At Gonstead Chiropractic & Wellness ATX, we do not use a one-size-fits-all routine. We evaluate how your spine and pelvis are functioning, ask about your pregnancy and symptoms, and coordinate with your obstetric provider when needed. Here is what Austin patients can generally expect during each trimester.

Before Your First Prenatal Chiropractic Visit

Your first visit begins with a conversation about your health history, due date, current symptoms, previous pregnancies, and any guidance from your OB-GYN or midwife. We also ask about complications, recent imaging, injuries, and activities that affect your discomfort.

The physical evaluation is adapted for pregnancy and may include posture, movement, spinal and pelvic function, and areas of muscular tension. We explain our findings and any proposed care before treatment. If your symptoms suggest a pregnancy complication or another condition outside our scope, we will refer you to the appropriate medical provider.

First Trimester: Establishing a Comfortable Baseline

During the first trimester, visible postural changes may be limited, but fatigue, hormonal changes, and early ligament changes can still affect how you feel. Some patients visit for a pre-existing back or neck problem, while others want support as their body begins to adapt.

Visits are typically gentle and conservative. We check comfort frequently and avoid unnecessary pressure on the abdomen. This is also a good time to discuss supportive sleeping positions, lifting habits, and movement strategies. Chiropractic care does not replace prenatal medical care, so continue all scheduled visits with your OB-GYN or midwife.

Second Trimester: Adapting to Posture and Pelvic Changes

As the abdomen grows, the center of gravity moves forward and the lower back may work harder. Hip or pelvic-girdle discomfort can become more noticeable, especially with prolonged sitting, walking, rolling in bed, or getting in and out of a car.

We adjust table positioning and use cushions or side-lying positions to keep pressure off the abdomen. Care may focus on specific areas of restricted movement and muscular tension rather than forceful or generalized manipulation. Recommendations may also include gentle mobility, supportive footwear, or practical changes to workstation and sleep setup, depending on your obstetric provider’s guidance.

Third Trimester: Prioritizing Comfort and Mobility

In the third trimester, increased abdominal weight and changing pelvic mechanics may make standing, sleeping, and transitions more challenging. Appointments are modified as your belly grows, with supported side-lying or other comfortable positions and shorter periods in any one posture.

The goal is to support musculoskeletal comfort and movement—not to induce labor, turn a baby, or promise a particular delivery outcome. We continue to reassess rather than repeating the same technique at every visit. Frequency depends on your symptoms, response, schedule, and overall health; there is no universal prenatal treatment calendar.

When to Contact Your Obstetric Provider First

Back pain can be common during pregnancy, but certain symptoms require medical evaluation. Contact your OB-GYN, midwife, or other obstetric provider promptly for severe or sudden pain, vaginal bleeding, fever, burning with urination, fluid leakage, regular cramping or contractions, reduced fetal movement, or symptoms that concern you. These are not situations to manage with a routine chiropractic appointment.

Research on spinal manipulation during pregnancy remains limited, so honest screening, pregnancy-specific positioning, and communication among providers matter. You are always welcome to ask why a technique is recommended and how it has been adapted for pregnancy.
